Appointment of a Health Care Representative (Medical Power of Attorney)
This document empowers another person to make medical decisions for you, but only if you are incapacitated and cannot act for yourself.

Living Will
A document that provides instructions that you do not wish heroic measures taken to extend your life in the event you are in a terminal medical condition.

Guardianship 
Appointed by a court of a Guardian to handle business affairs and/or to make decisions, such as healthcare decisions, for a person who is no longer capable of doing so.

The best time to have a plan in place for handling your affairs is before you have the need to use it.
